Funeral homes requesting Cremation Authorizations should fax the SIGNED Death Certificate to 239-277-5017. We will examine the Death Certificate and fax back an approval unless the Terminal Sequelae cannot stand alone on the Death Certificate, e.g., Pneumonia.
Cremation authorization fees have increased to $50 effective 6/1/15 per Lee County Ordinance 15-07.
Funeral homes will receive a statement after the end of each month.
